Duct Excision — Microdochectomy and Total Duct Excision

Duct excision surgery, includes microdochectomy, if a procedure performed to remove a single milk duct from the breast or total duct excision when all the ducts are removed.This surgery is typically recommended when there is persistent nipple discharge, particularly if the discharge is bloody or occurs without squeezing. The aim is to diagnose and treat the underlying cause, which could range from benign conditions like papillomas to more serious concerns like ductal carcinoma in situ (DCIS).

Why is duct excision surgery recommended?

Duct excision surgery is advised when there is persistent or unusual nipple discharge and imaging tests reveal abnormalities within the milk ducts.

Before the operation 

Before the surgery, your surgeon will conduct a thorough evaluation, including a clinical breast exam and imaging tests such as a mammogram or ultrasound. You’ll be given specific instructions, which may include fasting for a few hours before the procedure. Discuss any medications you are taking with your doctor, as some may need to be paused.

During the operation 

Duct excision surgery is usually performed under general anaesthesia. The surgeon makes a small incision near the areola to access and remove the affected duct/ducts. The procedure typically takes about 30 to 60 minutes. The removed tissue is then sent to a pathology lab for examination.

After the operation

Post-surgery, you may experience some soreness, swelling, or bruising around the incision site. Over-the-counter pain relief can help manage discomfort. Most patients can go home the same day, but it's important to follow your surgeon’s aftercare instructions, which include keeping the incision clean and avoiding strenuous activities for a few days.

Recovery time and results

Recovery from duct excision surgery is generally quick. Most people can return to their normal activities within a week. The results from the pathology lab usually come back within a few days to a week, and your surgeon will discuss these findings with you during a follow-up appointment.
